Survey: Collaborative practice in low vision

  • Greetings,

    You are invited to participate in a research study. The purpose of the study is to explore whether occupational therapists in rural settings participate in collaborative, interprofessional, low vision practice and how these services are carried out.

    We are inviting you to participate in this study because you are an occupational therapist and member of the American Occupational Therapy Association or your state occupational therapy organization.

    • The survey takes around 15 minutes to complete.
    • If you do not choose to participate in the research study, please ignore the link to the survey.  
    • If you choose to participate, you may skip any question you would prefer not to answer.

    We will keep the information you provide confidential; however, federal regulatory agencies and the University of South Dakota Institutional Review Board (a committee that reviews and approves research studies) may inspect and copy records pertaining to this research. 

    This survey does not ask for any personal identification information, in order to keep all participants’ responses anonymous. If this research is published, you will not be identifiable based on your responses to the survey. 

    All survey responses that are received will be treated confidentially and stored on a secure server. However, given that the surveys can be completed from any computer (e.g., personal, work, school), we are unable to guarantee the security of the computer on which you choose to enter your responses. As a participant in our study, we want you to be aware that certain "key logging" software programs exist that can be used to track or capture data that you enter and/or websites that you visit.

    There are no known risks from being in this study. You will not benefit personally from participating in the study; however, the study may benefit low vision treatment practice.

    You do not have to participate in completion of this survey. You do not have to answer any questions you do not want to answer. You may exit the survey at any time without penalty.
